For Troop Volunteers 
Accident/Incident Report: This form must be completed anytime an accident or incident occurs involving a member of Girl Scouts of Texas Oklahoma Plains.
Activity Approval Form: Use this form to receive council approval for high-risk activities, travel, and money earning projects. Submission is required for the activity to be considered a Girl Scout event.
Annual Financial Review: Each June, this form must be completed by the troop treasurer in order for the troop to be in good standing with Girl Scouts. The form can be completed online or via the Volunteer Toolkit.
Bank Letter Request: Are you a troop or service unit that needs to open a Girl Scout bank account, close a Girl Scout bank account, or change signers on an existing Girl Scout bank account? If you answered yes to any of these questions, complete the form below to start the process.

Day Trip and Activity Permission Form: Use this form to receive permission for day activities outside of a regular troop meeting.
Health History and Emergency Medical Authorization Form: This fillable form, for both girls and adults, helps ensure that leaders have all the pertinent information to keep Girl Scouts safe and healthy during events.
In-Home Girl Scout Meeting: Use this form to request permission to meet at an individual's private home for a Girl Scout meeting. Approval from GSTOP must be received before any meeting with girls in a private home can occur.
Media Release - Adult: Download and print this form for use with events that require a media (photography/videography) release outside of the general membership release.
Media Release - Minor: Download and print this form for use with events that require a media (photography/videography) release outside of the general membership release. These must be signed by a parent/guardian over the age of 18.

Board Member Referral Form: Use this form to refer a community member as a potential member of the Girl Scouts of Texas Oklahoma Plains' Board of Directors.
Chaperone Interest Form: Use this form if you are interested in serving as a registered, background checked adult chaperone.
Child Abuse and Neglect Prevention: GSTOP's policies and procedures workbook as it relates to child abuse and neglect prevention to keep every child safe.
Financial Assistance Application for Resident Camp or Retail: Financial assistance is limited; however, the council makes every effort to provide enough support so that no need-based new Girl Scout in Texas Oklahoma Plains is denied participation due to lack of funds. A notification of assistance status will be emailed within two weeks of receipt of a completed application.
Financial Assistance for Girl Scout memberships is requested when registering for a Girl Scout membership through the MyGS portal. Assistance is provided on an as-needed basis.

Progression Charts
Entrepreneurship Progression: Allows girls to gain new skills, build their cofidence, and develop and entrepreneurial mindset over time as they engage in the Girl Scout Cookie Program.
Highest Awards Take Action Elements: Girl Scouts learn something new and have opportunities to be challenged in different ways as they earn the Bronze, Silver, and then Gold Awards.
