Accident/Incident
Report: This form must be completed anytime an accident or
incident occurs involving a member of Girl Scouts of Texas Oklahoma Plains.
Activity
Approval Form: Use this form to receive council approval for
high-risk activities, travel, and money earning projects. Submission
is required for the activity to be considered a Girl Scout event.
Annual
Financial Review: Each June, this form must be completed by the
service unit treasurer in order for the service unit to be in good
standing with Girl Scouts. The form can be completed online or via the
Volunteer Toolkit.
Bank
Letter Request: Are you a Troop or Service Unit that needs to
open a Girl Scout bank account, close a Girl Scout bank account, or
change signers on an existing Girl Scout bank account? If you answered
yes to any of these questions, complete the form below to start the process.

Health History and Emergency Medical Authorization
Form: This fillable form, for both girls and adults, helps ensure
that leaders have all the pertinent information to keep Girl Scouts
safe and healthy during events.
Media Release - Adult: Download and print this
form for use with events that require a media
(photography/videography) release outside of the general membership release.
Media Release - Minor: Download and print this
form for use with events that require a media
(photography/videography) release outside of the general membership
release. These must be signed by a parent/guardian over the age of 18.
Service
Unit Membership Report: Download this report to see where your
Service Unit membership numbers are for membership year starting
October 1, 2025.
Service Unit Job Descriptions: Learn about the
roles of various volunteer leadership positions within a service unit.
Service Unit Team Application: Apply to be part
of your Service Unit team for a one-year term, June 1-May 31. All
registered adult members of GSTOP are elegibile to apply, including
current position holders seeking to reapply.
Trip Participation Agreement: This form is
required for girls attending a SU or Troop trip. The form must be
printed, signed by the girl and parent/guardian, and returned to the
volunteer trip leader.
